Responsibilities
• Provide home-based physical therapy to patients who have lost mobility, strength, and functionality• Work as part of a home health team to create and implement patient's plan of care• Observe movements, assess progress, and modify care plan as needed• Assist patients with exercises and hands-on therapy to manage pain and increase mobility• Educate and coach patients and their families on the plan of care and therapy goals
Requirements
• Graduate of an accredited Physical Therapist Program• Active PT license in Wisconsin• Minimum of 1 year of physical therapy experience, preferably in home healthcare• CPR certification (preferred)• Good clinical judgement, strong interpersonal skills, dexterity, and compassion
