Medical Social Worker

Position : Social Worker (MSW)

Job Qualifications :
Graduate of a Master's degree program in social work accredited by the Council on Social Work Education. May have a master's or doctoral degree in social work. Minimum of one year's experience in health care, with experience in home health care preferred. Demonstrates strong verbal and written communication skills, along with excellent organization skills. Possesses and maintains current CPR certification. Must be a licensed driver with an insured automobile in good working order. Job Responsibilities :
Assess psychosocial status and communicate findings. Conduct social evaluations and plan interventions. Maintain clinical records. Provide information and referral services. Serve as a liaison between patients/families and community agencies. Collaborate with agency personnel. Develop contracts with agencies as resources. Participate in training programs. Contribute to plan of care and case conferences. Assist in understanding social factors related to health problems. Participate in discharge planning. Perform other delegated duties.
