Program Overview Since 1999, LifeSet has helped more than 8,000 young people aging out of state custody or other care arrangements successfully transition to independence. Participation in the LifeSet program is voluntary. Specially trained LifeSet specialists meet with LifeSet participants at least once a week-and more often when needed-in community settings, including their homes, at school or on the job, at a doctor's office or wherever is most convenient for the young person. Specialists are available 24/7 to help the young adult. Young people typically participate in the program for 6 to 12 months, based on individual needs.
As a LifeSet Specialist, one provides individual counseling and case management services to clients in the home and community setting to assist clients with transitioning to adulthood, specifically obtaining and maintaining safe and suitable housing, continuing their education, learning independent living skills, obtaining and maintaining employment, building a support system and remaining free from legal involvement.
 
Posi tion Overview Carry a maximum caseload of 8-10 young adults Meet with the young adult a minimum of one time per week, increasing and decreasing as directed by the Clinical Supervisor on the basis of clinical need. Provide on call crisis support to the client 24 hours a day 7 days a week (alternating on-call duties).
Provide accurate and complete information to Clinical Supervisor in a timely manner in the event of a crisis situation. Implement suggestions of interventions provided by supervisor and/or consultant in a timely manner to be reflected in the next treatment plan cycle. Participate weekly in all supervision exercises to ensure the appropriate conceptualization and implementation of treatment. Provide all therapeutic services in accordance with Youth Villages' Mission and Values and LifeSet Model Principles.
Conduct on-going assessment of the young adult's needs from a strength focused, solution-based perspective.
Engage and align with the young adult and their support system to elicit full participation in treatment. Additional Information Schedule is flexible and non-traditional; based around the availability of youth and families served Applicants must have a valid driver's license, a personal vehicle to use for work purposes and auto insurance Community Based staff will be reimbursed for applicable mileage If clinical licensure hours are needed (LPC, LCSW,etc.), this position qualifies for gaining those as you work with young people Salary $51000 - $59000 / year based on education and clinical license
Requirements A Bachelor's degree in social services discipline with experience working with at-risk youth, adults, or families is required A Master's degree in a social services discipline is preferred Degrees that can be considered include: social work, drug and alcohol education or counseling, psychology, criminal justice, guidance counseling, or marriage and family therapy.
