**CONFIDENTIAL APPLICATION PROCESS**
MDS Coordinator:
The MDS Coordinator is responsible for the coordination and management of the Minimum Data Set (MDS) process for skilled nursing facility (SNF) residents. This includes working with nurses, therapists, and other healthcare professionals to ensure accurate and timely completion of MDS assessments, care plans, and other required documentation. The MDS Coordinator also plays a key role in ensuring that the facility complies with all CMS MDS regulations.

MDS Coordinator Essential Duties and Responsibilities:
Coordinate and manage the MDS process for all SNF residents
Work with nurses, therapists, and other healthcare professionals to ensure accurate and timely completion of MDS assessments, care plans, and other required documentation
Review and analyze MDS data to identify trends and patterns
Develop and implement MDS-related quality improvement initiatives
Ensure that the facility complies with all CMS MDS regulations
Provide training and education to staff on MDS procedures and requirements
MDS Coordinator Qualifications:
Registered Nurse (RN)(LVN)
4+ years of experience in a skilled nursing facility setting
2+ years of experience with the MDS process
Strong knowledge of CMS MDS regulations
Excellent organizational and communication skills
Ability to work independently and as part of a team
MDS Coordinator Preferred Qualifications:
Bachelor's degree in nursing or a related field
MDS certification
Experience with MDS software programs
Experience with quality improvement initiatives
