Job summary
We are currently seeking a dedicated Community Resource Specialist to join our team.You will focus on assisting older adults enrolled in the Program of All-inclusive Care for the Elderly (PACE).The purpose of this position is to provide services to meet the social and/or emotional needs of clients.

Responsibilities
• Assisting older adults in obtaining necessary resources for maintaining independence in their own homes.• Participating in the development of comprehensive care plans for clients.• Developing policies for providing social services to clients in compliance with regulations.• Providing health care coordination, pain/symptom management, crisis intervention, and counseling.• Maintaining a valid Kansas Driver's License and automobile liability insurance.• Meeting organization's insurability standard for Motor Vehicle Record (MVR).
Requirements
• Bachelor's degree in Social Work.• Knowledge and skills in health care coordination, pain/symptom management, crisis intervention, and counseling.• Possession of a valid Kansas Driver's License and automobile liability insurance.• Maintenance of a Motor Vehicle Record (MVR) that meets the organization's insurability standard.
