Seeking a Case Manager to join our interdisciplinary clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our Priest Drive Recovery Health Center.  Offering a $2,000 Sign on Bonus for New Hires!!! NEW Competitive Compensation  Full-Time Employed: 40 hours/week Location: Priest Dr and Broadway Rd in Tempe, AZ  Bilingual in Spanish is a plus (Additional Language Differential Pay Available) Full Benefits Package, including 401K Generous PTO/Sick Time (4+ weeks in year 1) No experience required with a bachelors degree in behavioral health - we will train you New grads welcome Duties may include: Assists clients with identifying treatment and recovery goals and provides supportive services to help clients reach their goals Completes assessments, crisis plans and individualized service plans Works collaboratively with the clinical team to engage, educate, communicate and coordinate care with member, their family, behavioral health, medical and dental providers, community resources and others in ensuring that all services prescribed in the individualized service plan are implemented
Conducts outreach and follow-up of services including, but not limited to, crisis interaction, missed appointments, and hospital discharges to ensure adequate resources are available and in place. Performs all case management functions associated with caseload including completing the assessment and service planning processes; including identifying the need for further or specialty evaluations Pursues best practice outcomes for person with mental illness including continuing education, employment, independent housing and community tenure Benefits & Wellness: Multiple medical plans - including a no premium plan for employees and their families Multiple dental plans - including orthodontia Financial well-being - 401(k) with a company match, interest free medical line of credit, financial education, planning, and support 4 Weeks of paid time off in the first year Wellness program Child Care Support Program Pet Insurance Group life and disability insurance Employee Assistance Program for the Whole Family Personal and family mental and physical health access Professional growth & development - including scholarships, clinical supervision, and CEUs Employee perks and discounts Gym memberships Tuition at GCU and University of Phoenix Car rentals Bilingual pay differential  Education/Experience: GED/High school diploma with a minimum of 1.5 years of behavioral health experience, or, An associates degree in the field of medical or behavioral, or A bachelor's degree (not in the field of behavioral health) and 1 year of behavioral health experience, or A bachelor's degree in the field of behavioral, or A master's degree or higher in the field of behavioral health Computer Skills: Microsoft Word, Excel, Outlook, electronic medical record, NextGen preferred Valid AZ Driver License.  Must have valid Arizona driver's license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health's driving policy Must have a valid Arizona Fingerprint Clearance card (Level 1) or apply for an Arizona Fingerprint Clearance card within 7 working days of employment Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions Basic understanding of the DSM-V preferred Certified Case Manager strongly preferred Must pass background check, TB test and other pre-employment screening Physical demands of this position are representative of those that must be met by an employee to successfully perform the essential functions of this job.
