Coordinate with the national leaders and regional program managers/leads as well as other stakeholders to create detailed implementation plans. This includes deadlines, milestones, processes, and risk mitigation protocols. Initiate and set goals for Health Care programs based on the organizations strategic objectives. Collaborate with the regional Medical Directors on Disease Management Programs that serve the unique needs of their region. Report detailed status updates to leadership and executive team quarterly and as requested. Develop program operation workflows and budgets and present them to the Director, Value Based Optimization. Define department job roles and responsibilities. Provide leadership to all Care Programs and staff to achieve the organizational goals and objectives. Share frequent feedback to the Care Programs team using the CASA framework. Set annual objectives and goals and review clinical performance of the team and their respective program goals. Build a strong team through open communication and by collaborating on decision-making responsibilities. Collaborate with the Marketing team to create marketing initiatives that increase awareness of the programs provided. Identify new and creative program ideas to implement through continuous research and participation in regional and national education conference offerings. Ensure that the Health Care Programs deliver a return in outcomes and contribute to efficient cost management. Collaborate with the payer in the delivery of joint program objectives. Participate in payer meetings as necessary. Support all LOBs and service line projects. Align Programs to meet the needs of internal and external providers, for example PHSO Affiliates Providers.
