We invite you to join our Hematology/Oncology team as a Specialty Nurse Navigator. As a Breast Specialty Nurse Navigator Hematology/Oncology, you will have the unique opportunity to partner with our patients receiving cancer care.
We're able to navigate them through their journey while partnering with other teams to ensure comprehensive care.
Your responsibilities will include: Primary contact/advocate for patients in a large multidisciplinary hematology oncology practice.Collaborate with care team to develop and facilitate individualized plan of care.Educate patient and caregivers about disease, treatment modalities, side effect management, and self-care measures.Coordinate all aspects of care including diagnostics, procedures, referrals, and treatments.Triage all communications including patient calls and emails, other departments, insurances, genomic testing organizations.Form strong relationships with all colleagues including physicians, nurses, clerical staff, and management across multiple departments and locations.Navigating patients through the continuum of care acting as a liaison and resource between the physician, other departments, and the patient.Establish specific, measurable, attainable, relevant, and time-specific nursing goals to guide the care planning process and promote self-reliance.Provide in-person clinic visits as needed at the initiation of care and throughout transitions of care throughout all our hematology/oncology clinic sites.Formulate custom care plans from assessment and clinical information demonstrating the application of clinical guidelines including standards of care.Provide disease and treatment-specific education tailored to meet patient's individual goals of care.Onsite rounding throughout treatment plan implementation as needed to provide needed in-person patient evaluation.Provide direct, hands-on nursing care as needed.Assist patients in establishing, modifying, or reinforcing a social support network, thereby promoting self-reliance.
Assist patients with social services needs by identifying resources in the community and referring to these programs.Collaboration with system-wide authorizations team and scheduling in other departments as needed to ensure the patient receives timely and comprehensive care.
