Job Overview Job Title: Physical TherapistLocation: Eureka, California Type of Assignment: Contract (13 weeks)Start Date: 4/21/2025Shift: 5x8 Hours, 40 hours per weekShift Hours: 08:00 - 16:30Pay Rate: $58 - $60 per hour Qualifications: Valid California PT License BLS Certification (must be through American Heart Association - AHA)Minimum 1-year recent experience as a Physical Therapist (PT) in a Skilled Home Health Position is for Home Health - Humboldt County CA and surrounding areasRatios: 1:1 (*May visit up to 6 patients per day; 30 productivity points standard)System does a good job grouping clients minimizing drive and windshield time. Occasional maximum radius of 50+ miles1 Weekend Shift requiredCandidate is expected to have their own transportation.Facility is open to 26 weeks contractRequires primary series COVID vaccination in addition to one booster
