GENERAL SUMMARY
Design, organize and manage all Personal Outcome Measures. Oversee all Res Hab goals for assigned consumers. Oversee implementation of Individual Habilitation Plans. Oversee Behavioral Support Plans for the Bronx. Interact with all disciplines in a professional manner.
DUTIES & RESPONSIBILITIES
ESSENTIAL DUTIES
Execute and convey to staff, agency and programs all policies and procedures.
Train appropriate staff as POM (Personal Outcome Measure) interviewers.
Adhere to the mission of Person Centered Planning in delivery of services.
Train all staff on Person Centered Planning concepts.
Obtain input on person centered outcomes.
Supervise SAS (Skilled Acquisition Specialist).
Ensure that SAS is creating appropriate Person Centered Goals related to ADL skills and Person centered Outcomes.
Supervise Bronx Behaviorist.
Attend ISP meetings as needed.
Construct valued outcomes with Managers and appropriate staff.
Ensure delivery of each Individual?s Person Centered Plan.
Ensure delivery of each Individual?s Program Plan. Provide training on aspects of plan when necessary.
Review Behavior support plans for the Bronx.
Meet monthly with Behaviorist to monitor personal outcome measures in relation to behavioral supports.
Observe individuals, review data and progress monthly and revise valued outcomes as needed.

Do observations regularly to ensure staff compliance with Res Hab Plans, Person Centered and Behavioral Support plans.
Look for continuity between Res Hab goals, Plans, ISP and Behavior Support plans.

JOB SPECIFICATIONS (Skills, Knowledge, Ability)
Valid driver?s license, related experience.
CULTURAL AND SOCIOECONOMIC CHARACTERISTICS:
Ability to interact with and complete assignments in a manner sensitive to the service population?s cultural and socioeconomic characteristics.
LANGUAGE SKILLS:
Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group situations to peers, consumers, families and other employees of the agency.
MATHEMATICAL SKILLS:
Ability to add and subtract two digit numbers, and multiplies and divides with 10?s and 100?s. Ability to perform these operations using units of American money and weight measurement.
REASONABILITY SKILLS:
Ability to apply common sense and understanding in carrying out detailed, but uninvolved written or oral instructions. Ability to deal with problems involving a few concrete variables in standardized situations.
EDUCATION & EXPERIENCE
Bachelor?s Degree, QIDP, three (3) years experience working with developmentally disabled, valid driver?s license, and excellent writing/organizational skills and computer skills required. Experience with Behavioral Support. Must be CQL (Council on Quality and Leadership) Certified and must maintain this certification up-to-date.
PHYSICAL REQUIREMENTS
WORKING CONDITIONS:
While performing the duties of this job, the employee is regularly required to communicate clearly and effectively. The employee must occasionally lift and/or move up to 25 pounds.
