Current license or valid permit to practice professional nursing in Texas. Certification in nursing specialty or healthcare quality (CPHQ) required.

Job Description:

In partnership with the Nurse Manager, assist in planning, coordinating, and implementing patient care in an assigned area/department to focus on consistent, quality care and improvement of care, safety, and satisfaction. Accountable for healthcare outcomes for a specific group of patients in an area/department through the application of research:based information to design, implement, and evaluate patient plans of care. Has 24/7 accountability to meet the needs of the patient, family, and physicians.

Specific Job Related Duties

Leadership and Interprofessional Collaboration
Demonstrates effective communication, collaboration, and interpersonal relationships with members of the care delivery team across the continuum of care Demonstrates the ability to coach, delegate, and supervise healthcare team members in the performance of nursing procedures and processes with a focus on safety and competence Provides leadership to the healthcare team to promote health, optimize patient engagement and prevent future decline including progression to higher levels of care and readmissions Demonstrates a leadership role in enhancing group dynamics and managing group conflict
Outcomes Manager
Assumes a leadership role within an interprofessional healthcare team with a focus on the delivery of patient:centered care and the evaluation of quality and cost:effectiveness across the healthcare continuum Collaborates with healthcare professionals, including nurse clinicians, physicians, advanced practice nurses, nurse managers and others, to plan, implement and evaluate improvement opportunitiesParticipates in a shared leadership team to make recommendations for improvement at the unit and Health System level Uses performance measures and data sets such as HCAHPS and nurse sensitive indicators to assess and improve the delivery of evidence:based practices and promote outcomes that demonstrate delivery of safe, timely, effective, efficient, equitable, and patient:centered care
Systems Analyst/Risk Anticipator
Applies just culture principles and the use of safety tools such as risk analysis Maintains working knowledge of regulatory standards (The Joint Commission, Centers for Medicare and Medicaid and applicable disease specific accreditation) and monitor for compliance addressing any issues identified. Facilitates modification of nursing interventions based on risk anticipation and other evidence to improve healthcare outcomes Evaluates patient handoffs and transitions of care to improve outcomes Information Management Uses information technology, analytics, and evaluation methods to: a) Collect or access appropriate and accurate data to generate evidence for nursing practice b) Collaborate to analyze data from practice and system performance c) Design evidence:based interventions in collaboration with the health professional team
