Position: Full-Time OR Part Time Primary Care Physician Job Type: Full-Time Schedule: Monday to Friday (No Weekends) OR Part-Time (No Weekends), Flexible Schedule Location: San Diego Area About Us: Aligned Geriatrics is a rapidly growing mobile medical practice that specializes in Geriatric Medicine. Our mission is to provide high-quality and compassionate care for seniors in the comfort of their homes or senior living communities. As a primary care provider with Aligned Geriatrics you will play a crucial role in improving the lives of older adults by delivering personalized healthcare services.
Physician Responsibilities: Conduct comprehensive primary care visits in patients' homes or senior living communitiesCollaborate with a multidisciplinary team to ensure holistic and patient-centered careProvide preventive care, manage chronic conditions, and address acute issues in a timely mannerCollaborate with and coordinate care amongst various community partners to ensure continuity of careDetermine and initiate specialty referrals when appropriate Order and interpret lab and imaging testingPrescribe medications including controlled substances when appropriateBuild meaningful relationships with patients and their families to promote trust and understandingCollaborate with staff at various senior living communities to ensure timely and coordinated patient care Qualifications: Medical Doctor (MD) or Doctor of Osteopathy (DO) degreeBoard-certified or board-eligible in Internal Medicine, Family Medicine, or Geriatric Medicine (preferred) Active and unrestricted California medical license Experience in Geriatric Medicine is a plusStrong communication and interpersonal skillsExperience with Epic (EHR) is a plus Experience with Microsoft Office is a plus Strong time management and organizational skillsComfortable working independently and in team based environments Compassionate bedside manner with a commitment to providing patient centered care.
