How You Will Make A Difference
:
Responsible to the Clinical Manager for instructions on case management and accountable to administration of patient care.

Will carry out selected aspects of the patient care activities to a group of patients in a manner consistent with the philosophy of Advocate Health at Home.

Assure and improve quality of care for patients.
:
Facilitate patient and family rights to receive quality cost effective care.
Position Requirements
:
License in the State of Illinois
:
2 years of med/surg clinical experience
:
Ability to learn and use a computer based scheduling and documentation system.

Ability to communicate professionally both verbally and through written reports.
:
Good interpersonal skills.
