The post holder will work under the guidance and supervision of the Lead Consultant providing predominately face to face consultations, in one of our community based clinics.
The clinics provide pre-booked appointments for adult patients referred to the service.
A range of gastroenterology conditions will be seen in the clinics, including, but are not limited to, dyspepsia, abdominal pain, food intolerance, irritable bowel syndrome, constipation, NAFLD, raised Ferritin.
Main duties of the jobYou will be required to:
Participate with the team to optimise performance and ensure the delivery of high quality evidence based care.Offer support and mentorship to junior colleagues, promoting good practice and a learning environment.Participate in clinical audit as per HH policy and procedure.Actively contribute to development and efficiency of the service.Be competent in managing common gastroenterology presentations.Optimise the medical management of this group of patients, in agreement with the patient and discharge them back to their registered GP.Refer to secondary care and/or other services as appropriate.Be a source of clinical expertise to less experienced staff, and work in partnership within a multi-disciplinary team as appropriate.Participate in clinical meetings, HH clinical governance agenda, training and education.Be responsible for any pathology or radiology results that are ordered by themselves and support team members in interpreting the same as needed.About usHarrow Health Community Interest Company (HH) began in 2007 by a group of Harrow GPs, whose sole aim was to ensure patients receive a high quality clinical care service within the community.

We now deliver a number of community services including ENT, Dermatology, Gynaecology, ADHD and Enhanced Frailty Services.
HH Community Services directorate has been awarded a contract to provide general Gastroenterology service on behalf of NWL ICB.
This is also a Right to Choose contract across the country.

Job responsibilitiesTo support the Gastroenterology team to deliver a high standard of evidence based care to those attending the service.To work with the team to maintain a high quality, clinically effective and cost effective primary care service.To provide a comprehensive service to the patients attending the Gastroenterology clinic including clinical examination, investigations as needed, diagnosis and management, and referral as appropriate.To ensure patients have consented to, and are fully involved in the implementation of their procedure/treatment plans.Interpretation of results where requested for individuals.If a prescription is needed, this must be in line with the NHS England and local guidelines and clear instructions available to the patients registered GP.To adapt styles of communication appropriately in order to promote empowerment, self-management and health education for patients.
Interpreters can be sought as needed.Using the EMIS system the consultation must be recorded in the patients records.
Where the patient is registered with a Harrow GP the clinician will have access to their GP records.
Proformas are set on the EMIS system and must be completed by the clinician.
These will be shared with all the referring clinicians.Liaise with other team members as appropriate.To identify actual or potential clinical risks within scope of practice and take appropriate action using, when required, the appropriate reporting structure.To be personally accountable for professional and ethical actions and practices within a framework that adheres to the GMC's requirements and HH policies and procedures.Person SpecificationQualificationsFull registration with the General Medical Council.Appropriate qualification and experience in Gastroenterology.Relevant Higher Degree e.g.
PhD, MD or other clinical qualifications.ExperienceExperience of working independently, with a minimum of 5 years experience post qualification.MB ChB or equivalent.MRCGP or equivalent.Up to date knowledge of DOH, NICE and local guidelines for the management of gastroenterology presentations to the service, including pharmacological measures.Competency in interpreting and acting on pathology, histology and radiology results.Awareness of local referral pathways.Teaching experience.Mentorship and appraisal experience.Infection Prevention and Control Procedures.Knowledge of use of IT / EMIS system.Excellent written and oral communication skills.Regular attendance at Gastroenterology updates.Experience in completing audits and assessments.Knowledge/use of the EMIS System.Personal QualitiesTo be a reflective practitioner.Committed to self-directed development.Aware of HH Clinical Governance agenda.Assertive, adaptable/flexible/energetic and enthusiastic.Good communication skills and able to be sensitive and empathetic in distressing situations.Ability to work under pressure.Committed team worker and excellent leadership and communication skills.Possession of current Medical Indemnity.Able to challenge accepted practice as appropriate.Develop links with other primary care services as needed.Disclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
