Responsibilities
In keeping with Dignity Health philosophy, the Care Giver Navigator provides Social Service activities through collaboration with the care transition team and community partners, coordinating, and facilitating available resources to assist the caregiver and patient through the continuum of care in the outpatient setting.

QualificationsMinimum Requirements: 2 years social work experience in clinical setting minimum. Masters in Social Work required. Current California Drivers license and car insurance required.
Evidence of skills in research, communications (both written and oral), organization, creativity, leadership and group facilitation.
Preferred
Computer experience and knowledge in the management of computer data desirable.

Home Health/Hospice/Community Resource experience preferred.

Special Skills:

Expertise in one of the following areas: a. Assessment/counseling for adjustment to health problems, disabilities or loss b. Treatment of anxiety/stress reactions c. Behavior of cognitive problems d. Chemical abuse/dependence e. Complex discharge planning f. Child abuse/neglect g. Adult abuse/neglect h. Treatment of depression i. Suicide assessment j. Financial assistance k. Grief intervention l. Counseling for victims of violent crimes m. Sudden trauma or death. Ability to relate and maintain a rapport with all levels of professional personnel.
